Agreement:
I understand that it is my responsibility to find transportation each week to and from class, and I will notify my instructors if there are any changes in my ability to be present. I understand that I am committing to the program for the full 10 weeks and will be an active and engaged participant during that period. Failure to attend, at minimum, 8 sessions will disqualify you from receiving a certificate of completion. I understand that by submitting this application I am not guaranteed a spot in the program’s class. We process many applications and cannot accept everyone. Priority will be given to those currently living in Gastonia/Gaston County, minorities, and women who either already began a startup or are in the process of creating their first business. I understand that I may be placed on a waiting list for the next scheduled 10-week class if possible. I understand that the program fee is $125, and if I am selected to be a part of the program, I am responsible for paying the amount. Partial payments, and ongoing installments may be allowed at the discretion of the program overseers.

Waiver:
I do hereby give permission to The Company Lab (CO.STARTERS), its agents, and others working under its authority (Launch Gaston), full and free use of video/photographs containing my image/likeness. I understand these images may be used for promotional, news, research and/or educational purposes. I hereby release, discharge, and hold harmless The Company Lab and its agents from any and all claims, demands, or causes of action that I may hereafter have by reason of anything contained in the photographs or video. I do further certify that I am either of legal age or possess full legal capacity to execute the foregoing authorization and release.

Agreement:
I understand that The Company Lab and Launch Gaston does not guarantee that I will be successful in my business. Furthermore, the benefits gained from participating in CO.STARTERS are a result of time, energy and dedication to the training program, and thus are not based solely on the completion of the coursework. I further understand and authorize The Company Lab to provide applicable information to the assigned management counselor(s). I understand that any information released to be held in strict confidence by him/her. No person will be excluded from participation in or otherwise subjected to discrimination in regard to services, programs and employment provided by The Company Lab based on race, sex, color, national origin, sexual orientation, disability, age or religion.
